Description

The LACE index identifies patients that are at risk for readmission or death within thirty days of discharge. It incorporates four parameters (length of stay, acuity of admission, co-morbidities, and ER department visits within the last six months). The LACE score reflects the predicted rate of readmission or death within thirty days of discharge.


Calculator Search/Filter Keywords

At the calculator level, eCalcs comes delivered with default keywords that can be leveraged in the "Search/Filter" field to enable Specialty or System driven calculator searching. The following calculator keywords are delivered by default for the LACE Index Scoring Tool calculator:

Calculator Configuration Options

Preference Name Valid Range Description
Publishing Checked / Unchecked When checked the calculator will display for users on the front end.
When unchecked, the calculator WILL NOT display for users on the front end.
Orderable Item Override (BLANK) / G-LACE “(BLANK)” indicates that the global configuration options for Orderable Item will be used.
“G-LACE” indicates that the calculator specific value will be used and can display the specific calculator name in the output.
Show Full Questions in Annotations True / False True will enable the display of the full calculator input question text (rather than Q1, Q2, etc.) in the result output annotations.
False will disable the display of the full calculator input question text and display the question numbers and answers only in the result output annotations.

AIDS Problem AIDS-defining opportunistic infection or CD4 < 200
Any Tumor Problem Solid tumors must have been treated within the last 5 years; includes chronic lymphocytic leukemia (CLL) and polycythemia vera (PV)
Cerebrovascular Disease Problem Any previous stroke or transient ischemic attack (TIA)
Chronic Pulmonary Disease Problem Chronic Pulmonary Disease
Congestive Heart Failure Problem Any patient with symptomatic CHF whose symptoms have responded to appropriate medications
Connective Tissue Disease Problem Systemic lupus erythematosus (SLE) polymyositis, mixed connective tissue disease, moderate to sever rheumatoid arthritis, and polymyalgia rheumatica
Dementia Problem Any cognitive deficit
Diabetes With End Organ Damage Problem Diabetes with retinopathy, nephropahty, or neuropathy
Diabetes Without Complications Problem Diabetes with no retinopathy, nephropahty, or neuropathy
Metastatic Solid Tumor Problem Any metastatic tumor
Mild Liver Or Renal Disease Problem Cirrhosis but no portal hypertension (i.e., no varices, no ascites) OR chronic hepatits
Moderate Or Severe Liver Or Renal Disease Problem Cirrhosis with portal hypertension (e.g., ascites or variceal bleeding); Endstage Renal Disease, Hemodialysis, or Peritoneal Dialysis
Peripheral Vascular Disease Problem Intermittent claudication, previous surgery or stenting, gangrene or acute ischemia, untreated abdominal or thoracic aortic aneurysm
Previous Myocardial Infarction Problem Any previous definite or probably myocardial infarction
