Description

Patient Health Questionnaire (both version 2 and 9).

Functionality

This is used as a tool determine the severity of depression. The questionnaire will initially be in a PHQ-2 format. Should the user select a number greater than zero for an answer, the questionnaire will switch to PHQ-9.
