Description

The Risk Stratification Scoring (CPCI) is intended to provide a score-based evaluation of the patient’s relative risk. The intake form generates an aggregate score based on the utilization and number of health conditions selected for the patient. The score is then categorized as part of the scoring output as either Low, Moderate, Moderate High, or High. The Utilization section of the intake form is a manual point of entry. The majority of the categorized health conditions listed as check boxes are delivered pre-mapped to Problem entries that can be modified during or after implementation.


Calculator Search/Filter Keywords

At the calculator level, eCalcs comes delivered with default keywords that can be leveraged in the "Search/Filter" field to enable Specialty or System driven calculator searching.

Calculator Configuration Options

Preference Name Valid Range Description
Publishing Checked / Unchecked When checked the calculator will display for users on the front end.
When unchecked, the calculator WILL NOT display for users on the front end.
Create Task For Requested Items True / False When set to True (default) will generate a configured task to the assigned SDOH provider including questions that need follow up from the SDOH.
When set to False there is no task activity generated with a submitted SDOH form.

DM, Complicated/Uncontrolled Result Patient has diabetes mellitus either complicated or uncontrolled
DM, Controlled Result Patient has diabetes mellitus considered controlled
Hemiplegia/Paraplegia Result Patient has hemiplegia or paraplegia
Homebound Result Patient is homebound
HTN Result Patient has hypertension
Hyperlipidemia/Hypercholesterolemia Result Patient has hyperlipidemia or hypercholesterolemia
Hypothyroidism Result Patient has hypothyroidism
Liver Disease Result Patient has liver disease
Lives Alone Result Patient lives alone
Mental Retardation Result Patient is considered polypharmacy (six or more routine medications)
MI Result Patient has had myocardial infarction
No Insurance Result Patient has no health insurance
No/Little Social Support Result Patient has no to little surrounding social support
Non-compliance Result Patient is considered non-compliant (has not been seen in the past month with 2 or more no shows and has chronic condition such as DM/HTN/CHF, etc.)
Obesity Result Patient is obese
Peripheral Vascular Disease Result Patient has peripheral vascular disease
Polypharmacy Result Patient is considered polypharmacy (six or more routine medications)
