Health Management Plan Component

Health Management Plans (HMP) provide a tool to prescribe and monitor periodic events related to a patient’s health maintenance and disease management.

It is important to remember that although the HMP component may appear to be displaying all sorts of data, it consists of only three specific items.

• Orders or results linked to a problem • Prescriptions (medications) that are linked to a problem • Order reminders
