Health Management Plan
Introduction
The Health Management Plan or HMP component of the Clinical Desktop is a tool used to prescribe and monitor events related to a patient’s health maintenance and disease management. It enables users to view and modify health maintenance items for the selected patient. For instance, if a patient has Asthma, a doctor might utilize the HMP module to review the patient’s albuterol dose, confirm the patient has had an influenza vaccine, monitor the patient’s symptoms in a flowsheet, and setup a yearly reminder for a flu shot.
The Health Management Plan webcast defined the Health Management Plan or HMP component of the clinical desktop and reviewed the purpose and different components. 6 Main Views of HMP Component
There are six main views in the HMP Component
- Health Management - The Health Management view is a valuable tool that allows the user to quickly review and manage problems, current medications, reminders, and recent results for the selected patient
- Reminders/Alerts - The Reminders/Alerts view displays all active Order reminders placed on the patient’s chart.
- Flowsheets - The Flowsheets view is used to easily view data related to a specific visit type or disease management.
- Normative Growth Chart - The Normative Growth view displays growth data supplied by the Center for Disease Control for individuals 20 years and younger. This allows the provider to compare the patient’s percentages to the CDC standards.
- Immunization - The Immunization view is a comprehensive view of all historical vaccines for a patient.
- Vital Signs - The Vitals Signs view is a flowsheet of the patient’s vitals data.

Q&A
Q: Can you print the HMP? Is there a way to print the Health Management Plan from the ACI? I see where you can print flow sheets, but the Print function is greyed out for the HMP component so it is not a choice. Printing would be nice. It would allow providers to have a list of outstanding items needing be addressed at an appointment. Yes the physician should be looking at the screen, but there are many who do not take the computer into the room with them.
A: Currently, there is not functionality to print the HMP component from the clinical desktop. I’ve seen enhancement requests on ClientConnect for this, so hopefully it will be a part of 11.3. You can; however, print by using local printing: i.e.: print preview, “only print selected frame” and 100% in landscape and it prints okay. This could be a workaround until able to print from EHR. An additional option if using the Order Reminder tasks would be to print all Order Reminders within an Order Reminder task list.
Q: I upgraded from v10 to v11 and none of my “Last Done” dates are displaying for my order reminders. How can I re-enter this data?
A: If results were scanned into the system for these orders or the orders came over the interface, you can reference chartviewer to get the Last Done date. If you want to update the Order Reminder, you will need to right click in the To Do field and choose “Completed On”. A window will display that will allow you to choose a date in the past in which the order was created.
Q: How can we satisfy the completion of non-interfaced items such as a foot exam (referral), opthalmology exam, etc? I would presume that this could be done when the consult letter/result is being scanned in.
A: If an item is not interfaced, there needs to be workflow in place once the consult letter or exam letter is resulted. Once results come back, the user would go to the HMP and right click the "To Do" field and selecting "Last Done" and putting in the date that is on the result.
Q: We have not been using HMP since our upgrade from V10. What is your best suggestion for making the HMP a user friendly tool again with out the clutter of information that is presently there?
Am I understanding this that we will have to update over 50000 records by hand?
A: Collapse the problems list to make it a little more manageable. You can set a preference in the preferences menu under the HMP category to do this automatically. One thing to remember for V10 to v11, the "Last Done" date doesn't come over. One option is to look into chartviewer for the last result that came over for that item. If its an item that is only monitored by HMP, then that last done date would need to be found in ChartViewer, then youwould need to go to HMP to populate that last done date manually.
Q: Are there any scenarios in which deferring a reminder would be useful (from the to do column)? I have not been able to think of a good example to train our physicians.
A: You wouldn't defer a reminder, you would defer an order. An example would be: a patient can't come in the day of their appointment where a medication had been ordered, that medication would then be deferred for a later date.
Q: How do I create a graph on more than one resulted item?
A: First, you need at least two data points for a result in order to graph, but you check the items you want to graph in the graph column and then press the graph button.
Q: What is the text in dark red letters above the HMP?
A: These are the settings that are set up through HMP preferences. It is indicating which filters are being applied to a particular item type.
Q: What if a provider doesn’t want to see other provider’s (such as a specialist’s) HMP information that they have edited? If there a preference that would help drive that?
A: Collapsing problems that don’t pertain to a provider’s specific specialty would be a good way of limiting information you see from the patient’s chart that a provider isn’t responsible for. But there is no preference in Enterprise now that turns this off. You can organize by specialty type as well.
