MEDITECH’s presentation co-presented with Dr. Michael Fletcher, “Make Population Health a Reality: Effective Care Transitions,” will take place on March 2nd from 1:45 - 2:15 p.m. in the Population Health Knowledge Center. The presentation will review how care coordination is an essential element to improving outcomes for patients, payors and providers. We will go over new roles, processes, and tools that are emerging to promote effective care coordination and efficient care transitions, ultimately reducing re-admissions, increasing outcomes, and providing an overall better patient experience. We will discuss how technology will play an integral role as organizations manage larger clinical teams and patient populations to help improve overall population health.
