A Personal Health Record (PHR) is a medical record owned and maintained by an individual. Access to a person's PHR is controlled by that person.
The information contained in a PHR is often contributed to by an EHR. A series of EHRs may contribute to a single PHR via a RHIO. Some examples would include ihealth, CapMed and MyPHR
