Overview
Population Health Management (POP Health Management) is new 11.2 functionality and relates to HMP items. POP Health Management allows patients to be clinically tracked and placed into groups for management (i.e. males over 50 who haven’t had a colonoscopy). This tool exists in the VTB and can be added as a menu item from within TWAdmin. The functionality allows users to search patient populations, generate a query based on a patient population, and create or edit action sets (i.e. women over 40 who need an annual mammogram can have Health Management Reminders in their chart). From within these functions the user can refine searches by filtering to conduct a narrow, comprehensive search of their patient population. It is important to know that Population Health Management runs through the event bus and can update data within the data base; therefore, users should be utilizing this functionality during patient off hours at least during the initial stages after implementing the Upgrade, as it may cause performance issues.
