The Lower Extremity options on the joint selector noteform erases portions of the Plan from the note output

This seems to be a bug that has been identified in V11.4.1 HF 11 and 13, but it could be the noteform itself that is causing the problem. Would recommend putting a ticket into AHS for this issue.
